Inspector Notes

An unannounced monitoring inspection was conducted on November 28, 2023, from 9:40am-10:45am. At the time of entrance, ( 8 ) children ( 25 ) points were in care with ( 3 ) caregivers present. The inspector reviewed 5 children?s records, the caregiver and household records are current. Children were observed playing with toys and coloring. No medications were on site. Areas of non-compliance are identified on the violation notice.

Based on a review of children records, 1 child's record does not have documentation of a completed physical examination within 30 days after the first day of attendance.
Evidence: Child # 2, date of first attendance 7/31/2023 does not have documentation of a physical examination on file.
Plan of Correction: The provider contacted the parent, they will bring in a copy of the most recent physical examination.

Based on a review of children records, 5 of 5 records reviewed did not have documentation of the person who viewed and the date that the child's proof of birth was viewed.
