Inspector Notes

An unannounced monitoring inspection was conducted on Thursday, June 29, 2023, to determine the provider's compliance with licensing standards. The inspector was on site from 10:55am to approximately 12:40pm. During the inspection, there were a total of nine children in care, totaling 20 points, in the direct care of three caregivers. Upon arrival, the children were observed playing in their designated play area outside. Later, the children were observed washing hands, during diaper changes, taking bathroom breaks, and eating lunch. All areas used for childcare were inspected. The family day home has age and stage appropriate materials and supplies. The caregivers were observed having positive interactions with the children in care. The first aid kit and non-emergency supplies were inspected and found complete. Per the provider, there are no firearms in the home, transportation is not provided, and medication is not administered at this time. Four caregivers' records, one household member's record, five children's records, and pet vaccination records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of five children's records, the provider did not obtain documentation of additional immunizations every six months for a child under the age of two years.

Evidence: 1) The most recent immunization documentation in the record of Child #1 (22-months-old), enrolled on 08/28/22, was dated 08/24/22.

2) During interview, the provider confirmed the family day home has not obtained additional immunizations since the child was enrolled.
Plan of Correction: Additional immunizations will be requested today.
