Inspector Notes
An unannounced monitoring inspection was conducted on Thursday, March 7, 2024, to determine the provider's compliance with licensing standards. The inspector was on site from approximately 10:20am until 12:30pm. During the inspection, there were a total of 12 children in care, totaling 31 points, in the direct care of three caregivers. Upon arrival, the children were observed in their designated play area , they were observed during free-play, dancing, and singing. The children and caregivers later transitioned to the playground. Diapering and hand-washing procedures were observed, as well as the children eating lunch. The caregivers were observed having positive interactions with the children in care throughout the inspection. All areas used for childcare were inspected. The family day home has age and stage appropriate materials and supplies. The first aid kit and non-emergency supplies were inspected and found complete. Per the provider, there are no firearms in the home, transportation is not provided, and medication is not administered at this time. Four caregivers' records, one household member's record, five children's records, and pet vaccination records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
5Evidence: The results of the fingerprint-based national criminal background check in the record of Caregiver #4, employed on 11/20/23, were completed on 01/17/24. During interview, Caregiver #1 acknowledged the fingerprints were obtained after Caregiver #4 started employment.
Evidence: The sworn statement in the record of Caregiver #4, employed on 11/20/23, was completed on 12/01/23. During interview, Caregiver #1 acknowledged the sworn statement was completed after Caregiver #4 started employment.
Evidence: The physical examination in the record of Child #2, date of attendance 04/14/23, was completed on 08/08/23. During interview, Caregiver #1 acknowledged the physical examination for Child #2 was not obtained within 30 days after the first day of attendance.
Evidence: The emergency response drills for 2023 were reviewed. An emergency evacuation drill was not documented in November 2023. During interview, Caregiver #1 reported that an evacuation drill was not practiced in November as required.
Evidence: The immunizations in the record of Child #2, date of attendance 04/14/23, were obtained on 06/03/23. During interview, Caregiver #1 reported the immunizations for Child #2 were requested prior to the child's attendance, but were not obtained until after the child attended the family day home.
