Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a tour of the program.
A complaint inspection was initiated on July 29, 2021 and concluded on September 14, 2021. A complaint was received by the department regarding allegations in the areas of Special care provision and Emergencies and Staffing and Supervision. The staff in charge was contacted by telephone to conduct the investigation. The inspector interviewed and reviewed documentation required to complete the investigation.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
1Evidence : Observation , staff interview and centers attendance computer program confirmed on July 29, 2021 there were children ages 2 years to 9 years in the butterfly classroom and the classroom was not in ratio. The ratio in the classroom with youngest being two years old should be 8 children to 1 staff. At approximately 7:10 am there were 1 staff with 10 children and 7:20 there were 1 staff with 12 children the youngest in the classroom was 2 years old confirmed by the center staff. The next staff arrived for the classroom was at approximately 7:35am.
In the cuddle bugs room , there were 2 staff with 9 children at 7:25 am. The youngest in the classroom was 7 months old. The ratio for the children birth to the age of 16 months is one staff member for every four children. Staff confirmed the group was not in ratio at 7:27pm.
Confirmed with staff there were only three staff in the building between 7:10am-7:35am.
Staff has been added to infant room at 7:00am. PreSchool staff has been added at 7:15.
Assistant director is now on site at 7:30am and on call between 6:30am-7:30am.
