During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated on 5/7/2025 and completed on 5/7/2025 in response to a complaint received by the licensing office on 5/6/2025 relating to allegations of unsafe physical plant and sick child policies. This inspection included document review, tour of the facility, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

The center had a non-operable shed that had exposed rusty nails and splintered wood within the playground area that school age children played.
