Based on a review of caregiver records, the Provider did not orient the assistant by the end of the first week of assuming job responsibilities.

Based on observation, the bathroom did not contain paper towels for hand washing.

Evidence:
1. There were no paper towels available in the bathroom for hand washing at the time of the inspection.

Based on observation, the provider did not ensure that the total number of children receiving care at any one time did not exceed the maximum licensed capacity of the home.

Evidence:
1. At the time the inspector arrived to the family day home (at approximately 10:15am), there were 10 children (27 points) in care. The family day home is only licensed to provide care for up to 5 children.

Based on observation, all children were not provided an individual crib, cot, or rest mat for resting or napping.

Evidence:
1. There were 3 cribs observed that were not identified for individual use at the time of the inspection.

Based on a review of children's records, 3 out of 3 records reviewed did not contain the required information.

Evidence:
1. Child #1 (doe:10/25/2022), Child #2 (doe: 8/1/2022), and Child #3 (doe: 8/24/2022) did not have documentation of verification of proof of ID on file at the time of the inspection.

Based on observation, caregivers did not ensure that children's hands were washed with liquid soap and warm running water.

Evidence:
1. 3 children in highchairs were given a snack to eat and were observed eating the snack without washing their hands.

Based on a review of child records, 1 out of 3 children did not have documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.

Evidence:
1. Child #2 (doe: 8/1/2022) did not have documentation of immunizations on file at the time of the inspection.

Based on observation, children were allowed to drink while lying down.

Evidence:
1. Upon arrival to the family day home, Child #3 (infant) was observed lying down drinking a bottle in a crib.

Based on observation, high chairs with safety waist and crotch straps were not fastened according to the manufacturer's instructions.

Evidence:
1. There were 3 children observed in highchairs without any of the buckles fastened during the time of the inspection.

Based on observation, bottles were not labeled with the child's full name and the date when more than one infant is in care.

Evidence:
1. A bottle was located on the shelf in the living room with no name/date on it and a second bottle was located in a crib with no name and date on it. There were two infants in care at the time of the inspection.
