Inspector Notes
An unannounced monitoring inspection was conducted today between 10:15am-11:15am. There were 10 children present with the Provider and 1 assistant for a total of 27 points. The Provider was over capacity at the time of the visit. The licensed capacity of the family day home is 5 children. The children were observed having a snack, being held, and 4 children were picked up prior to the end of the inspection. A complete inspection of the family day home space, children and caregiver/household member records, evacuation drill log, and attendance were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation. 13Evidence:
1. Caregiver #2 (doh: 4/6/2023) did not have documentation of fingerprints background check results at the time of the inspection.
Evidence:
1. Caregiver #2 (doh: 4/6/2023) did not have documentation of 2 of 2 required references of character prior to employment.
Evidence:
1. Caregiver #2 was unable to communicate with emergency personnel at the time of the inspection.
Evidence:
1. Caregiver #2 (doh: 4/6/2023) did not receive orientation (no documentation) training by the end of the first week of assuming job responsibilities.

1. There were no paper towels available in the bathroom for hand washing at the time of the inspection.

1. At the time the inspector arrived to the family day home (at approximately 10:15am), there were 10 children (27 points) in care. The family day home is only licensed to provide care for up to 5 children.
Evidence:
1. There were 3 cribs observed that were not identified for individual use at the time of the inspection.
Evidence:
1. Child #1 (doe:10/25/2022), Child #2 (doe: 8/1/2022), and Child #3 (doe: 8/24/2022) did not have documentation of verification of proof of ID on file at the time of the inspection.
Evidence:
1. 3 children in highchairs were given a snack to eat and were observed eating the snack without washing their hands.

1. Child #2 (doe: 8/1/2022) did not have documentation of immunizations on file at the time of the inspection.
Evidence:
1. Upon arrival to the family day home, Child #3 (infant) was observed lying down drinking a bottle in a crib.
Evidence:
1. There were 3 children observed in highchairs without any of the buckles fastened during the time of the inspection.
Evidence:
1. A bottle was located on the shelf in the living room with no name/date on it and a second bottle was located in a crib with no name and date on it. There were two infants in care at the time of the inspection.
