Inspector Notes
An unannounced focused inspection was conducted today between 12:10pm-1:20pm. There were 7 children (19 points) present with the Assistant caregiver. The Provider was not present during the inspection. The children were observed eating lunch and taking a nap. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, evacuation drill log, menu, attendance and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation. Violations
3Evidence:
1. There were 7 children (19 points) with 1 caregiver during the time of the inspection. The Provider was not present during the inspection and the Assistant was over her points.
Evidence:
1. Child #2 did not have an Information for Parents form and Provisions of the Emergency Preparedness Plan form filled out at the time of the inspection.
Evidence:
1. The menu for lunch on the day of the inspection listed "chicken breast w/quinoa mix, vegetables & strawberries." What was being served for lunch at the time of the inspection was noodle soup and milk. There were no substitutions listed on the menu.
