During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 3/25/2025. The on-site inspection began at 12:10pm and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 7 children present and 1 caregiver; the point total was 15. The inspector reviewed 5 children?s records and 4 caregiver/household
members records on-site on 3/25/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the
violation notice issued to the program.
Violations
1of identification of the child within 7 days of enrollment.
Child #3 did not have documentation of verifying the proof of identification of the child
on file at the time of the inspection. The child had been enrolled for more than 7 days.
