During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 12/19/24. The on-site inspection began at 10:50am and ended at 11:43am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver; the point total was 8. The inspector reviewed 2 children?s records and 2 caregiver/household member(s) records while on-site. The inspection included document review, tour of the daycare areas of the home and interview. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

The record for Child #7 did not include documentation of a physical exam. The child has been enrolled over 30-days.
Plan of Correction: The provider said the child has a doctor's appointment scheduled in January and she will receive the physical exam at that time.
Standard 8VAC20-800-60-B
A child's record shall contain the name, address and phone number of each parents place of employment.

The record for Child #8 did not include the work information for each parent. Those spaces were left blank on the enrollment form.
Plan of Correction: The provider said she will ask the parent to provide this information. The child began attending today.
