Inspector Notes

An unannounced monitoring inspection was conducted on 4/13/22 from 10:15am - 12:00pm. During the inspection there were 11 children ages five years old through twelve years old in care with 2 staff. Children were observed participating in various activities in the swimming, eating lunch and playing outside. Records were reviewed for five children and three staff. Medication, emergency procedures, and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Based on a review of three staff records and interviews, it was determined that the facility did not ensure that there shall be at least two staff members who meet the requirements of subsection A of this section present on the premises during the center's hours of operation, on fieldtrips, and wherever children are in care.

Evidence:
1. The Licensing Inspector reviewed the records for both staff present and only staff #1 had a current CPR/First aid certification.
2. Staff #1 confirmed that staff #2 did not have a current CPR/First aid certification.
Plan of Correction: The facility responded: Staff #2 will complete the current CPR/First Aid certification. We will ensure that there is always two staff present with a current CPR/First Aid certification.

Based on observation and interviews, it was determined that the licensee did not ensure that when food is brought from home it is labeled with the child's name and date.

Evidence:
1. The Licensing Inspector observed that none of the lunch boxes the children had brought from home were labeled with their name or the date.
2. Staff #1 acknowledged that all of the lunch boxes used by the children at the facility were not labeled with their name and the date.
Plan of Correction: The facility responded: Going forward we will ensure that all food from home is labeled with the child's name and date.
