During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/12/25. The on-site inspection began at 10:10am and ended at 12:45pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 16 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/22/25. A POC submitted after this date will not appear on the public website.

The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 6/24/25, were not posted.

Staff shall complete a minimum of 16 hours of training annually, appropriate to the age of children in care. Staff #1 obtained five of the 16 required hours in 2024.

If sunscreen is used it shall be labeled with the child's name. There were 4 bottles of sunscreen that were not labeled with the child's name.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include all of the required information. There were five injury reports that did not contain all of the required information.

When food is brought from home the food container must be clearly dated and labeled in a way that identifies the owner. There were three water bottles that were not labeled with the child's name and date.
