A Subsidy Health and Safety Inspection (SHSI) was conducted on 3/7/2024 from 3:45pm to 5:25pm. A licensing inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Based on record review and interviews, it was determined the vendor did not ensure that staff who work directly with children shall, in addition to preservice and orientation training required, they shall annually attend at least 16 hours of training and staff development activities, to include the departments health and safety update course.
