Inspector Notes

A Subsidy Health and Safety Inspection (SHSI) was conducted on 8/13/24. A licensing inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/1/24. A POC submitted after this date will not appear on the public website.

Based on record review and interviews, it was determined that the vendor did not ensure that all staff who work directly with children shall have within 90 days of the date of employment or 90 days from subsidy vendor approval, current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
1.The record for staff #3 (date of hire 6/6/17) did not include current certification in CPR. The CPR in the record expired 8/2023.
2.The record for staff #6 (date of hire 8/16/22) did not include current certification in CPR. There was no documentation is the record that CPR certification had been completed.
