During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was completed on (date), as a part of the licensure period. The on-site inspection began at 12:00pm and ended at 2:30pm. The inspector reviewed compliance in the areas listed above. There were 60 children present and 9 staff. The inspector reviewed 4 children?s records and 5 staff records. This inspection included: document review, tour of the facility, interviews, observations and measurements. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-150-B
Each immunization report shall be signed by a physician, his designee, or an official of a local health department. Child #1, #2 and #3 did not have an immunization report with a physician's signature indicating they were "adequately immunized."
Plan of Correction: The director will contact parents to obtain a signed immunization report for each of the children.
Standard 8VAC20-780-260-A
The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official. The last fire inspection of the center was dated 03/24/2025.
Plan of Correction: The center has an appointment with the fire marshal to have the inspection completed by the end of the month.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe condition. There was a broken electrical outlet cover in the main daycare area and an electrical cord plugged into the wall and on the end was a hair dryer between two water sources.
Plan of Correction: The director will contact the custodian to replace the broken outlet and the director will ensure that prior to children using locker rooms that it checked for any hazards such as plugged in electrical appliances.
Standard 8VAC20-780-320-B
Each restroom area provided for children shall be equipped with soap, toilet paper, and disposable towels or an air dryer within reach of children. A school-age child walked out of a bathroom to look for another bathroom because it did not have any toilet paper.
Plan of Correction: The director said she would contact the custodian to resupply the bathrooms.
