A subsidy health and safety inspection was initiated on 6/30/22 and concluded on 6/30/22. There were no children present the day of the inspection. The provider has the same children in care from the previous inspection and no updates are needed at this time for children?s records. The provider?s records were reviewed for updates. The physical plant was observed and the provider was interviewed regarding policies and procedures.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the facility.

Based on record review and interview, the provider failed to undergo TB screenings at least every two years from the date of the previous screening.
