Inspector Notes

An unannounced monitoring inspection was conducted today between 10:50am and 12:30pm. There were 6 children present with the Provider and 1 assistant within the supervision guidelines. The children were observed transitioning from water play outside, playing in the playroom, eating lunch and other organized activities. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard 8VAC20-770-40-D-4-a
Based on record review and interview, the provider has not obtained a sworn disclosure statement for each household member over 18 years old.
Evidence: Household member #1, Household member #2 and household member #3's records do not contain sworn disclosure statements. The provider confirmed that the household members have not completed the background check.
Plan of Correction: We'll get them done today.
Standard 8VAC20-770-40-D-5-a
Based on interview, the provider did not obtain a central registry background check for a person residing in the home within 30 days of becoming 14 years old.
Evidence: The provider confirmed that a Household member #4 turned 14 years old in April 2022. The provider stated that they did not complete a central registry background check for the household member.
Plan of Correction: I'll have them fill out the form and I think we can get it notarized and in the mail this weekend.
Standard 8VAC20-770-60-B
Based on record review and interview, each employee has not completed a sworn disclosure statement.
Evidence: The provider's record does not contain a sworn disclosure statement. The provider confirmed that they had not completed the background check.
Plan of Correction: I can fill it out today.
Standard 8VAC20-800-100-A
Based on record review, the provider has not obtained documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #1 (date of enrollment: 3/2/2021), Child #2 (date of enrollment: 3/2/2021), Child #3 (date of enrollment: 4/26/2021) and Child #5's (date of enrollment: 7/29/2021) records do not contain documentation of a physical exam.
Plan of Correction: I'll request the health form from the parents.

Based on record review, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently.
Evidence:
1. Caregiver #1's most recent TB screening is dated 3/10/2020.
2. Household member #1's most recent TB screening is dated 3/10/2020.

4. Household member #3's most recent TB screening is dated 3/10/2020.
Plan of Correction: We'll make the appointments and we'll all get them updated.
Standard 8VAC20-800-320-A
Based on observation, small electrical appliances were not unplugged when not being used by the caregiver or with children under close supervision.
Plan of Correction: I didn't know. I'll unplug them.
Standard 8VAC20-800-60-B
Based on record review, children's records did not contain all required information.
Evidence:
1. Child #1, Child #2 and Child #3's records did not contain a second emergency contact.
2. Child #1, Child #2 and Child #3's records did not contain an address for their emergency contact.
Plan of Correction: I can update my forms and get it corrected. I'll have the parents give me the missing information.
Standard 8VAC20-800-70-A
Based on record review, parents were not provided in writing all required information.
Evidence: 5 out of 5 children's records did not contain documentation signed by parents whether or not the family day home has liability insurance.
Plan of Correction: I'll create the form and have the parents sign and return it.
