Inspector Notes
An unannounced monitoring inspection was conducted today between 10:50am and 12:30pm. There were 6 children present with the Provider and 1 assistant within the supervision guidelines. The children were observed transitioning from water play outside, playing in the playroom, eating lunch and other organized activities. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

8Evidence: Household member #1, Household member #2 and household member #3's records do not contain sworn disclosure statements. The provider confirmed that the household members have not completed the background check.
Evidence: The provider confirmed that a Household member #4 turned 14 years old in April 2022. The provider stated that they did not complete a central registry background check for the household member.
