Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 1/8/2025. The on-site inspection began at 11:20 a.m and ended at 12:30 p.m. The inspector reviewed compliance in the areas listed above. There were 13 children present and 3 caregivers; the point total was 37. The inspector reviewed 4 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, authorization forms), a tour of the family day home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

Child #2 (date of enrollment: 9/3/2024), child #3 (date of enrollment: 8/7/2024) and child #4 (date of enrollment: 5/28/2024) records do not contain documentation of a physical exam.

REPEAT VIOLATION
Potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children.

Various cleaning agents were observed in the cabinet under the sink in the children's bathroom. The lock on the cabinet was hanging open during the inspection.

The bathroom shall be kept clean and contain a working toilet and sink, toilet tissue, liquid soap, and paper towels.

The childrens bathroom did not have paper towels.

High chairs shall be used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened.

Four out of four children observed eating lunch in high chairs were not strapped into the chairs with the safety straps.
