During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on /4/2025. The on-site inspection began at 1:30 PM and ended at 2:30 PM. The inspector reviewed compliance in the areas listed above. There were 13 children present and 3 caregivers; the point total was 37. The inspector reviewed 5 children?s records and 3 caregiver/household member(s) records. This inspection included document review, a tour of the family day home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. The family day home shall obtain subsequent background check results for each caregiver every five years.

Caregiver #1s most recent fingerprint background check is dated 5/15/2020. The background check has been expired for approximately 3 weeks.

The family day home shall obtain a central registry background check for each caregiver within the first 30 days of employment.

The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

No soft bedding of any kind shall be used under or around infants including, but not limited to, pillows, quilts, comforters, sheepskins, or stuffed toys.

Child #1 (15 months) was observed sleeping in a crib with a pillow.

REPEAT VIOLATION
Swings shall be used according to the manufacturers instruction and when occupied by a child, a safety strap shall be used and securely fastened.

Child #12 was observed asleep in a swing. The safety strap was not being used to buckle the child into the swing.

Standard 8VAC20-800-570-A
The provider shall ensure that a caregiver does not exceed 16 points by using the following point system to determine if an additional caregiver is needed:
1. Children from birth through 15 months of age count as four points each;
2. Children from 16 months through 23 months of age count as three points each;
3. Children from two through four years of age count as two points each;
4. Children from five years through nine years of age count as one point each;

Caregiver #1 and caregiver #3 were observed supervising 13 children (37 points) without a third caregiver. The caregivers were over the supervision requirements by 5 points for approximately 30 minutes during the inspection.

SYSTEMIC DEFICIENCY
The family day home shall obtain documentation of additional immunizations every 6 months for children under the age of 2 years.

Child #8s (15 months) most recent immunizations are dated from then the child was 6 months old. Updated immunizations have been overdue for approximately 3 months.
