Inspector Notes

A complaint inspection was conducted on June 9, 2022 from 10:20am to approximately 12:45pm in response to a complaint regarding physical abuse of a child by staff. The complaint was received by the licensing office by March 9, 2022. Staff were interviewed, documentation was reviewed, and a tour of the facility was conducted. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program and were discussed during the exit interview.

Based on observation and interview, it was determined that the center did not ensure that staff did not use physical punishment on children.
Evidence:
1. There is video footage of Staff #1 throwing a ball at Child #1 as punishment.
2. Staff #1 was immediately terminated.
3. Staff #2 confirmed that Staff #1 was terminated immediately.
Plan of Correction: The center responded with the following: Staff #1 was immediately terminated from the center. There are all new staff members at the facility. The center will ensure that appropriate discipline training is provided to all new staff.
