Inspector Notes
A subsidy inspection was conducted on July 18, 2022 from 11:35am to approximately 1:00pm. At the time of arrival there were eight children in care and four staff members. A religious exempt code compliance inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program. The violations are listed on the violation notice issued to the center and were reviewed with the site director at the exit interview.

Based on record review and observation, it was determined that the vendor did not ensure that there be at least one staff on duty who has obtained within the last three years instruction in performing a daily health observation of children.
Evidence:
1. There are no staff members that have instruction in performing a daily health observation of children.
2. Staff #1 confirmed the training has not been provided nor do the staff members at the center have the daily health observation training.

Standard 8VAC20-790-640-A
Based on observation and interview, it was determined that the vendor did not ensure that hazardous substances such as cleaning materials be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. A bottle of ant and roach insecticide was found on the kitchen counter with the warning label, "Hazard to humans" while children were in care.
2. Staff #1 confirmed the bottle being on the kitchen counter.

Plan of Correction: The vendor responded with the following: During the inspection the bottle of insecticide was immediately removed and locked. Staff will ensure that all hazardous materials will be locked and kept away from children.
