During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 07/09/2025, as a part of the licensure period. The on-site inspection began at 9:00 AM and ended at 11:00 AM. The inspector reviewed compliance in the areas listed above. There were 14 children present and 2 staff. The inspector reviewed 3 children?s records and 3 staff records on-site.
This inspection included:

? tour of the facility,
? interviews, and
? observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3The facility did not conduct a practice emergency evacuation drill in June 2025.
Emergency evacuation drills will be conducted and documented monthly.
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include all required documentation.
An injury record for child 4, di not include the child's name, date and time of the injury, staff present, the date, time and how the parent was notified of the child's injury, any future action to prevent recurrence of the injury and the signatures of two staff or a parent and a staff person.
In the future, all children's injuries will be recorded on an injury report form that includes all required documentation. All staff will be re-trained regarding injury reporting and written records will be monitored by administrators.
Child 4 sustained an injury requiring outside medical attention on June 23rd and the department's representative was not notified until July 3rd.
In the future, the department will be notified of any serious children's injuries requiring outside medical attention within 2 days.
