During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 07/17/2025, as a part of the licensure period. The on-site inspection began at 12:05 PM and ended at 2:00 PM. The inspector reviewed compliance in the areas listed above. There were 28 children present and 4 staff. The inspector reviewed 4 staff records on site.
This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening prior to coming into contact with the children. Staff 1 and 2, both on duty with the children during the inspection, did not have documentation of negative tuberculosis screenings.
Plan of Correction: Provider response:
We will ensure that all staff obtain negative tuberculosis screenings within 30 days prior to employment and prior to coming into contact with the children
Standard 8VAC20-780-70
Staff records shall be kept for each staff person that includes the job title, date of employment, documentation to demonstrate that the individual possesses the education, certification and experience required by the job position and the required orientation training.
The records provided for staff 1 and 2 did not include the job titles, dates of employment and the required orientation training. the records provided for staff 3 and 4 did not contain documentation of the education, certification and experience required by program leaders.

We will ensure that all staff records contain clear documentation of the date of employ, job title, the required orientation documentation and documentation of their qualifications for the position (program leaders).
