Standard 63.2-1509-A
Staff employed by a child day program who have reason to suspect that a child is abused or neglected shall report the matter immediately to the local department's toll-free child abuse and neglect hotline.
Staff that were aware of an incident related to neglect of a child (2 years old) in care, did not report the incident to the local department.
Plan of Correction: The incident was reported to the Licensing Inspector within 48 hours and was followed by a required report to CPS. At this time, the investigation has been completed and the case is now closed.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.
1. On 11/5/25, Child #1 (2 years old) was left alone and unsupervised on the playground for approximately two minutes, with no staff to ensure the child's care and protection.
2. Video footage showed that Staff #1 lined up 7 children from the 2.5 year old classroom against the fence to bring them in.
3. At 10:00:45 Child #1 who was at the end of the line, withdrew from the line and slid into the play equipment. Staff #1 did not see that and walked inside with 6 children at 10:01 am.
4. Staff #1 stated that she had counted the children when they were lined up and also when they were inside.
5. Video footage showed that at 10:03:27 am, Staff #2 who had taken another child to the bathroom returned to the playground, thinking that the children and Staff #1 would still be there.
6. When Staff #2 walked into the playground, Staff #2 saw Child #1 standing alone. Staff # 2 walked around and checked the playground to see if any other children or staff were present. There were none.
7. At 10:04:18, Staff #2 took Child #1 back inside to the office to management staff.

Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff.
1. On 11/5/25 at approximately 10:00 am, Child #1(2 years old) was left alone and unsupervised by Staff #1 for two minutes when she took 6 children inside the building from the playground.
2. Staff #1 had lined up 7 children from the 2.5 year old classroom against the fence to bring them inside. Child #1 was at the end of the line.
3. At 10:01 am, as Staff #1 walked inside with the other children, Child #1 slid away from the line into the play equipment.
4. Staff #1 did not see that Child #1 slid away, and closed the door once the other 6 children had walked inside.
5. Child #1 was found by Staff #2 at 10:03:27 when Staff 2 returned to the playground.
Plan of Correction: The teacher lined up the children and completed a head count before leaving the playground. However one child pulled away unnoticed and hid behind the playground equipment. The teacher responsible did not follow the proper procedures for transitioning children indoors and as a result their employment was terminated. Following the incident all staff members in the building were retrained on outdoor supervision procedures including strict name to face accountability during every transition. We reinforced expectations for active supervision, frequent scanning. and consistent child counts during all movement from one area to another. Children's safety is our highest priority, and we take every incident seriously. Our commitment included maintaining vigilant supervision, ensuring staff understand and follow all safety protocols and implementing immediate corrective actions when procedures are not followed. We remain dedicated to providing a secure environment where every child is accounted for, protected and supported at all times.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
1. On 11/5/25, daily attendance was not accurately documented in writing or electronically for the 2.5 yr old classroom.
2. The written log for the classroom did not have attendance documented for the children in care for all days leading up to 11/5/25 and including 11/5/25.
3. The center uses an electronic software that documents attendance. Staff # 3 stated that the parents log in children as they arrive.
4. A review of the electronic attendance for 11/5/25 showed: a. two children logged in when they were not present for the day; b. one child that was transitioned to another room was still logged in as being present; and c. 4 children that were not logged in by the parents were logged in all at 9:19 am by Staff #2, which is not the time that they all arrived.
Plan of Correction: Moving forward, teachers will check children in upon arrival using Brightwheel and paper attendance. Parents will no longer be required to complete the check-in process. This change will help us maintain more accurate attendance records.
