Inspector Notes
An unannounced renewal inspection was conducted on March 2, 2023. There were 33 children present with 9 staff supervising. The physical plant, children's records, staff records, emergency drill logs, attendance logs, allergy list, menu's, evacuation preparedness procedures, bottles, cribs and medication and authorizations were reviewed. The children were observed working on lessons, listening stories and playing with floor toys. Areas of non-compliance were cited on the violation notice. Violations
5Evidence:
1. In the Jr. Toddler room, a blue chair has cracked on the front.
2. In the Jr. Toddler room, the changing table is soiled.
The changing table will be cleaned today.
Evidence:
In the Infant room, in the nap area, there is an outlet cover missing.
Evidence:
In the Jr. Toddler classroom, the changing pad had a hole in it and therefore is no longer nonabsorbent.
Evidence:
In the Infant room, there were bottles and food containers that had children's name, but were not dated.
Evidence:
Child #3 and #5's records did not contain documentation of the work phone number of each parent who has custody.
