An unannounced inspection was initiated on 9/30/2025 and completed on 9/30/2025 with a local agency, in response to a complaint received by the licensing office on 9/26/2025 relating to allegations of Forbidden Actions. The inspector reviewed children?s records and staff records on-site on 9/30/2025. This inspection included document review, a tour of the facility, interviews,
and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
