Inspector Notes

The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 7/19/2022 relating to the physical plant, supervision and the center offering children three years of age or younger foods to that are considered a choking hazard. Interviews were conducted with staff, inspected the classrooms, reviewed staff to children ratio documents and reviewed the center?s menu.

The information gathered during this investigation does not support the allegations relating to the physical plan or supervision.

The information gathered during this investigation does support the allegations of the center offering children three years of age or younger foods that are considered choking hazards, so the complaint is determined to be valid. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 5 business days of receipt. Based on review of the center's menu, the center is offering foods that are considered to be potential choking hazards to children three years of age or younger.
Evidence:
1. The center's menu list raisin bran cereal for AM snack, raisins with string cheese on 7/14 for PM snack on 7/18 and provides to all children besides infants, Raisins are listed as a choking hazard on the USDA list.
Plan of Correction: Change menu to omit raisin
