Inspector Notes

A self-reported incident was received by the department on 9/02/2022 regarding leaving a toddler in a classroom alone while the teachers and remaining children went to the play ground. An investigation was initiated and concluded on 9/14/2022. Staff were interviewed and two staff files and one child's file was reviewed.
Information gathered during the investigation of the self-report determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-340-F
Based on interviews, it was determined that the center did not ensure that all children 10 years of age or younger shall always be within actual sight and sound supervision of staff.
Evidence:
1. On 9/02/2022, Child #1 (Age 20 months), was not within actual sight and sound supervision of staff for approximately 5-6 minutes.
2. Staff #3 and Staff #4 stated that they were walking by the Jr. Toddler Class on their way to the playground and saw that Child#1 was alone in the classroom. Staff #3 took the child to the playground where the Jr. Toddler Class was playing and spoke with Staff #1.
4. Staff 1 and Staff 2 were not able to explain how Child #1 (20 months old child) was able to leave their supervision.
Plan of Correction: The eachers of the class were re-trained on the policy "Policy_Child Supervision
_Accident Prevention" on 9/14/2022. They have signed off for committing to alway
follow the policy to count children at every transfer threshold (stair landing, door step, playground gate, before leaving, after arriving).

Additionally, all staff members were alerted and followed through the incident and undertood the severity of the incident.
Standard 8VAC20-780-40-E
Based on staff interviews, the center's activities and services were not maintained in compliance with the center's own policies and procedures that are required by these standards.
Evidence:
1. The center's policy on supervision states: ?Staff should regularly count children in care, at every transition, and whenever the group is leaving one area and arriving at another to confirm the safe whereabouts of every child at all times.?
2. On 9/02/2022,Staff #1 and Staff #2 of the Jr. Toddler Class did not count children when they arrived at the playground as required by policy:
*At approximately 10:00am, Staff #1 and Staff #2 lined up 9 children in the Jr. Toddler classroom to go outside. They stated they counted the children before they left.
*When they arrived at the playground, both teachers stated they did not do a count of the children.
