Inspector Notes
The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 9/20/2022 relating to the menu and not being informed of substituted food and that a teacher was yelling at children in the 2 year old classroom. Interviews were conducted with staff, a document summarizing a meeting with staff was reviewed, daily meals checklist from the catering services and menu were reviewed.
The information gathered during this investigation does support the allegations, so the complaint is determined to be valid. See the Violation Notice for findings.

Violations
4Evidence:
1.Staff # 2 and Staff #4 work together in the Sr. Toddler class. Staff #2 stated ?[Staff #4?s] tone scared me and the kids at times?; and that Staff #4 ?verbally started yelling at me in front of the kids because I had reported her for not following the curriculum.?
2.In a document summarizing meetings with Staff #2 and Staff #4, Staff #1 wrote, ?In the morning of Thursday 9/15/2022 both [Staff #4 and Staff #2] were arguing in the classroom, loudly, which led to Staff #1 stepped in the classroom to understand what was happening [sic].?
which led staff 4's subsequent resignation as she refused to sign off the warning letter.
Evidence:
1.Staff # 2 and Staff #4 work together in the Sr. Toddler class. Staff #2 stated Staff #4 would ask children ?why are you like this?? ?I heard [Staff #4] yell at the children several times.? Staff #2 stated Staff #4 ?speaks very loudly.? Staff #2 also stated ?Staff #4 would drag the children by the arm to time out.? Staff #2 stated ?Staff #4 was aggressive and would grab the children by the arm.?
2.Staff #1 stated while in the hallway, she could hear Staff #4 yelling at a new child that was crying and spoke with her about it 2-3 months ago.
Evidence:
1.Staff #1 admitted the center changed the menu on 9/06/2022 and 9/07/2022 and did not notify the parents. Staff #1 stated they usually email the parents of any changes, but did not for those two days.
2.The Daily Meal checklist from the catering company for 9/6/2022 lists chicken nuggets. The center?s menu listed Cheese pizza for 9/6/2022. For 9/7/2022, the center menu listed Fish Filet, but the children were served cheese pizza.
Evidence:
1. The center did not have the findings of the inspection dated 9/14/2022 posted on the bulletin board during the inspection.
