Inspector Notes

A self-reported incident was received by the department on February 12, 2024, regarding a staff member grabbing a three year old child by the arm by force to get them off from the play structure. Interviews were completed with staff and documents were reviewed.

The information gathered during this investigation of the self-reported incident determined the allegations were not valid.

Any violations not related to the self-report, but identified during the course of the investigation can be found on the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Based on review of one staff record, the center did not obtain the central registry finding within 30 days of employment.
Evidence:
1. Staff #1 (Date of hire 12/06/2023) had a central registry finding dated 1/18/2024 and was more than 30 days of hire..
Plan of Correction: The staff was given the orientation on 12/6/2023 with a start/hiring date
in January 2024. We'll no longer provide orientation prior
to the start date

Based on review, orientation training for one staff member did not include all the facility specific topics.
Evidence:
1. Staff #1 (date of hire 12/06/2023) had orientation training on 12/06/2023, but did not include the topic of shaken baby syndrome and abusive head trauma, including procedures to cope with crying babies and distraught children.
Plan of Correction: Staff orientation training is now using VDOE model form
to better document and track trained items and to avoid missing
procedures
