Inspector Notes
An unannounced monitoring inspection was conducted on 4/29/2024 from 9:25am to 11:55am. At the time of entrance, 62 children were in care with 11 staff members present. Children were observed during Montessori individual learning time, playing outside, sitting in highchairs eating snack, playing with toys in the infant classroom, washing hands and diaper changes. (6) staff records, and (5) children?s records, the physical space, evacuation drills, medication, the injury prevention plan and attendance records were reviewed. The site was clean and organized. Interactions between the children and staff were positive. Walls and bulletin boards were decorated with artwork. Areas of non-compliance are identified in the violation notice. Repeat violations were found and are identified within this report.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

1. Staff #4's orientation documentation was missing the delivery method and the entity or individual providing training.
Evidence:
1. In the Jr. Toddler Classroom, one yellow wood chair had a large chip in the wood on the back of the chair and another yellow wood chair back was coming apart (the peg was out of the slot).
Based on observation, hazardous substances were not kept in a locked place using a safe
locking method that prevents access by children.
Evidence:
1. In the Sr. Toddler classroom, a spray bottle of bleach water was on the wall basket and not kept in a locked place.
2. In the Hall bathroom, a spray bottle of Febreze was next to the sink. The door to the bathroom was open.

Evidence:
1. In the Jr. Toddler Classroom, the emergency contact list for children was posted on the board near the teacher desk that included names, phone numbers and relationship to the child.
Based on observation, the center did not meet all diaper ointment or cream requirements.
Evidence:
1. In the infant classroom, two diaper creams were in low cubbies and were within reach of children and accessible.
Evidence:
1. The center documented one shelter in place drill on 4/27/2023, but did not document a 2nd drill in 2023.
