During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated and completed on 2/24/2025 in response to a self-report received by the licensing office on
2/10/2025 relating to a serious injury that occurred at the center.
The Department issued a Special Order on August 22, 2025 to require the licensee to
contact parents, guardians, or other responsible persons (families) regarding health and safety
violations as a result of high-risk violations.
This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/04/2025. A POC submitted after this date will not appear on the public website.
Violations
3Documentation of orientation training shall be kept by the center in a manner that allows for identification by individual staff member, is considered part of the staff member's record, and shall include all required documentation information.
Staff #1's who has been working 5 months at the center had orientation training documentation that was missing the training delivery method.

audits of staff files to ensure completeness of files to prevent
future documentation errors.
When staff are supervising children, they shall always ensure their care, protection, and guidance.
On 2/10/2025 at approximately 12:10pm, Child #1 (12 month old infant) received a serious injury while Staff #1 was washing the infants hands at the food preparation area sink. The infant reached over touching a crock pot that is used to heat bottles and burnt the infant in two areas on the back of the left 5th digit. The infant received second degree burns at the base of the finger and dorsum of hand and at mid finger joint.

An accident form was filled out for Child #1 (12 month old infant) for an injury that occurred at the center on 2/10/2025. The accident report was signed by one staff member and was missing another staff signature or parent signature.
