During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 9/03/2025

The Department issued a Special Order to place Licensee on Probation and require licensee to
contact parents, guardians or other responsible persons regarding health and safety violations on 7/11/2025 as a result of high-risk violations.

The on-site inspection began at 12:45pm and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 56 children present and 6 staff. This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 9/10/2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
The center shall maintain a written record of children's serious and minor injuries in which
entries are made the day of occurrence and include all required information. The record shall include treatment and documentation on how parent was notified.

An accident report dated 7/08/2025 was missing the method of how the parents were notified of the injury and the treatment of the injury.
Plan of Correction: Staff have been reminded that all accident reports must include
treatment provided and the method of parent notification. A refresher
training on completing injury reports will be conducted at the next staff
meeting. Administration will review all reports daily to ensure they are
complete before being filed

Repeat Violation
For each group of children, the center shall maintain a written record of daily attendance
that documents the arrival and departure of each child in care as it occurs.

In the infant classroom, 4 children were in the classroom and 3 children were listed on the classroom attendance record.
In the Senior Toddler classroom, 13 children were in the classroom and 6 were listed on the classroom attendance record.
In the Jr Toddler classroom 9 children were in the classroom and 10 were. Listed on the classroom attendance record.
Plan of Correction: Staff were reminded to record attendance at the time of each
child?s arrival and departure. A mandatory refresher training on
accurate documentation will be held at the next staff meeting.
Lead teachers will check attendance twice daily, and administration will
conduct daily spot checks. Attendance sheets will be reviewed at the end of
each day to ensure accuracy.
