During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 10/21/2025.The Department issued a Special Order to place Licensee on Probation and require licensee to
contact parents, guardians or other responsible persons regarding health and safety violations on 7/11/2025 as a result of high-risk violations.

The on-site inspection began at 9:55am and ended at 11:48am. The inspector reviewed compliance in the areas listed above. There were 57 children present and 9 staff. The inspector reviewed 5 children?s records and 5 staff records on-site electronically on 10/21/2025.. This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

There was a carpet near the door to go outside was lifted and rolled from the floor causing a tripping hazard
Plan of Correction: A replacement carpet has been installed on the same day.
Standard 8VAC20-780-280-B
Hazardous substances must be in a locked place that prevents access by children.

In the Sr. Casa classroom, a spray bottle of Windex and Simply Saline Nasal Mist were on a shelf near the cots.
Plan of Correction: Teachers were reminded to lock hazardous subtances.
The Windex and Nasal Mist were locked in the closet right away.

Repeat Violation
Staff and children's records shall be treated confidentially.

In the Jr. Toddler Classroom, a child contact list with the parents names and telephone numbers were posted on the teacher's bulletin board without a cover sheet.
Plan of Correction: Teachers were reminded to cover parent information. A cover sheet
has been added right away.
