Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. An out-of-state child abuse and neglect registry search must be requested prior to employment.

Staff #2, employed for 6 weeks, did not have documentation of requesting a central registry check from one state prior to employment. The request was sent two days after employment. The center has not obtained the results.
Plan of Correction: The Center will from hereon ensure to complete all required background checks before getting a new employee onboard.

We have sent a follow-up email on 12/23/25 to the concerned state for the unreceived clearance.
Standard 8VAC20-780-240-E
Within 30 days of the first day of employment, staff must complete orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.

Staff #1 and Staff #2 had been employed more than 30 days and did not have documentation of completing orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.
Plan of Correction: For new employees, we will make sure that the orientation checklist is diligently gone over with to ensure that all aspects of it are oriented to the new employee

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following record shall include the following staff and parent signatures or two staff signatures; and documentation on how parent was notified.

Child #1's injury report was missing the method of how the parent was notified and missing two staff signatures or staff and parent signatures.
Plan of Correction: Staff have been reminded that all accident reports must include all details as required by the form. A refresher training will be included in the next staff meeting. Administration will make sure to double check all reports and ensure that all information are complete and accurate.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

In the Infant classroom, the attendance record documented the arrival of 2 children being present, however only 1 child was present in the classroom. One child left the classroom to visit another classroom and staff did not document the child's departure from the classroom.

In the Sr. Toddler classroom, .the attendance record documented the arrival of 10 children being present, however only 8 children were present in the classroom. Two children left the classroom to visit another classroom and staff did not document the children's departure from the classroom.
Plan of Correction: Staff has been writing names of transitioning kids to another class on the white boards close to their doors.
It has been explained to the staff the importance of ensuring all kids physically present in the room are accounted for in the list. Administration will check the Attendance list daily to ensure that this is being done correctly.
Standard 8VAC20-820-120-E-6
A copy of any special order issued by the department shall be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.

The Special Order dated 7/11/2025 was not posted in the center in a prominent place of each entrance of the licensed premises.
Plan of Correction: The Special Order has always been there posted. Administration took it at some point to review and was not able to put back. The Special Order has been posted back in the Parent Information Board.
