Inspector Notes

A mandated renewal inspection was initiated was conducted on December 13, 2022 from 9:50 A.M.-3:00 P.M. There were 182 children present, ranging in ages from two months to six years of age, with 35 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 14 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-240-A
Based on review of staff records, the center failed to ensure that the Virginia Department of Education orientation course was completed within 90 days of employment.

Evidence:
1. Staff #4's date of hire was 05/06/2022. The orientation training on file was dated 10/08/2022. The training was required to be completed by 08/06/2022.
2. Staff #5's date of hire was 05/24/2022. There was no documentation in the file that the orientation training had been completed. The training was required to be completed by 08/24/2022.
3. Staff #6's date of hire was 05/02/2022. The orientation training on file was dated 10/11/2022. The training was required to be completed by 08/02/2022.
4. Staff #10's date of hire was 03/25/2022. There was no documentation in the file that the orientation training had been completed. The training was required to be completed by 06/25/2022.
Plan of Correction: Administration will ensure that all staff will complete the Preservice Training within the 90 day timeframe.
Standard 8VAC20-780-520-A
Based on review of over-the counter skin products, the center failed to ensure that non prescription skin products shall be kept beyond the expiration date of the product.

Evidence:
1. In Classroom #1 there was a container of Boudreaux's Butt Past that expired in July 2020.
2. Staff verified the expiration date on the bottom of the container.
Plan of Correction: Product removed from the classroom. A new policy was created that includes administration reviewing over the counter skin products forms and medications before entry into the classroom.
