During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated on March 5, 2025 and completed on March 28, 2025 in response to a complaint received by the licensing office on February 27, 2025 relating to allegations of supervision. The inspector reviewed one child?s record and one staff record on-site on March 5, 2025. This inspection included document review and interviews

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 7, 2025 . A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care, protection, and guidance.
Child #1, four years old, was sleeping on a cot during nap time on February 26, 2025. Staff #1, vacuuming the floor with a battery operated floor sweeper, sucked up strands of Child #1?s hair, tearing it from the scalp and leaving a bald spot.
Plan of Correction: We have taken immediate corrective actions to prevent future occurrences. The use of vacuums and floor sweepers is now strictly prohibited while children are present, and all such equipment has been removed from classrooms. We have implemented a new vacuum policy to ensure all floor cleaning occurs outside of classroom hours, and staff will only use brooms while students are present. Staff have been retrained on supervision, safety protocols, and proper cleaning procedures. Additionally, management will conduct regular compliance checks to ensure adherence to these policies.
