Inspector Notes

A monitoring was conducted on 6/23/22 from 9:50am until 10:50am. At the time of the inspection, there were 04 children in care with 2 staff present. A sample of 5 children's records and 2 staff records were reviewed. Children were observed participating in morning learning activities and engaging in free play. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and required center postings were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law. Violations are listed on the violation notice issued to the center and were discussed with the center director during the exit interview.

Based on observation and interview, the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. The wooden lattice barrier covering the air conditioning unit on the playground is broken. There is also exposed torn wire mesh with pointy sharp edges under the lattice barrier.
2. The center director confirmed that the wooden lattice barrier coving the air conditioning unit on the playground is not in a safe and operable condition.
Plan of Correction: The center responded with the following: The wooden lattice barrier for the air conditioning unit will be repaired as soon as possible.

Based on a review of emergency supplies and interview, the center did not ensure that one working, battery-operated flashlight and one working, battery-operated radio is kept at the center.

Evidence:
1. The center does not have a battery-operated flashlight and does not have batteries for the battery-operated radio.
2. The center director confirmed that the center does not have the required battery-operated flashlight or batteries for the battery-operated radio.
Plan of Correction: The center responded with the following: The flashlight for the center was mistakenly taken to the other center. The flashlight will be returned to the center today and batteries for the radio will be purchased.

Based on record review and interview, the center did not ensure that children's records contain all the required information.

Evidence:
1. The record for child 1 (date of enrollment: 6/17/22) does not contain the addresses of the two designated people to call in an emergency if a parent cannot be reached.
2. The center director confirmed that the record for child 1 is lacking the addressed of the two emergency contacts.
Plan of Correction: The center responded with the following: The addresses for the emergency contacts for child 1 will be obtained today at pick-up,
