Documentation shall be maintained of emergency evacuation, shelter-in-place and lockdown drills. There was no documentation that emergency evacuation drills had been practiced for the months of October and November 2025 and no documentation that shelter-in-place and lockdown drills had been practiced within the last year.
Plan of Correction: The center responded with the following: The practice evacuation drills for October and November has been conducted but not documented on the practice drill log. Shelter-in-place and lockdrown drills had also been conducted but not documented. Going foward, the center will make sure to document each drill as they occur on the practice drill log.
