Areas of the center, inside and outside, shall be maintained in a safe and operable condition. On the playground, the door to the wooden gate is broken and does not close or latch. There are also rusted bolts and protruding nails on the bottom of the gate door.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. In the kitchen, there were numerous cleaning materials in an unlocked cabinet under the sink. The cleaning materials had warning labels stating ?keep out of reach of children?, ?may irritate eyes? and ?do not swallow? and were accessible to the children in care.

During the stated hours of operation, there shall always be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member with direct means for communication between the two of them. At the time of the inspection, staff 1 was alone in the center supervising 4 children, ages 3-4 years, who were napping. Staff 1 stated this is a regular occurrence, as staff 2 takes her scheduled break from 12:00pm until 2:00pm, leaving staff 1 alone to care for the children.
Plan of Correction: The center responded with the following: A third teacher was now hired so there will always be two teachers in the building.

Children under 10 years of age shall always be within actual sight and sound supervision of staff. During the inspection, staff 1, who was the only staff person in the building, left 4 napping children, ages 3-4 years, alone in the center to go outside and retrieve a child off the school bus. The children were unsupervised for approximately 3 to 4 minutes. Staff 1 stated she regularly leaves the children inside the center to go outside to meet the school bus as she is the only staff person in the building during this time.
