Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was conducted on 4/12/21 and 4/13/21. The provider was contacted by telephone to initiate the inspection. There were four children (10 points) and two caregivers present . The entire home and outside play area along with the equipment to be used by the children was inspected. The Inspector reviewed 2 children?s records and 2 caregiver records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Violations
1Evidence:
1. The record for staff #2) did not contain documentation of a completed the search of the central registry and the sex offender registry check from the state of North Carolina. The sworn statement or affirmation located in the record for staff #7 indicated that staff #7 lived in the state of North Carolina within the last 5 years.
2. Caregiver #1 reviewed the record for the caregiver #2, and confirmed that the required out-of-state background checks were not received for caregiver #2.
