Inspector Notes

An unannounced monitoring inspection was conducted on 05/10/2022 from 12:40 pm - 4:00pm. At the time of entrance there were a total of 109 children were present with 13 teaching staff during nap time. Children were observed during nap and transiting from nap to afternoon program time. Two diaper changes were observed in the infant classrooms. Records were reviewed for five children and six staff.
Violations were found in the areas of children's records, hand washing and staff background checks. These violations are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.

Based on record review and interview, the center failed to obtain the results of a criminal record information check from any state in which the individual has resided in the preceding five years.

Evidence:
1. Staff 1, hire date 12/15/2021, lacks the results of a criminal record check from a state she has lived in within the past five years.
2. Administrative staff confirmed that an out-of-state criminal record check had not been requested.
Plan of Correction: The program director stated that she thought she had obtained a criminal record check when the sex offender check and central registry check was obtained from this state.
During the inspection a criminal record check from this state was requested.
All other required background checks are on file for staff 1.

Based on record review and interview, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.

Evidence:
1. Child 1, enrollment date 10/18/2021, lacks a documented physical exam on file.
2. Administrative staff confirmed that a physical exam was not on file for child 1.
Plan of Correction: The program director provided the following plan of correction:
The parent of child 1 was contacted during the inspection and asked to bring in a copy of the physical exam.

Based on observation, the center failed to ensure that children's hands are washed with soap and running water after toileting.

Evidence:
1. After completion of a diaper change, at 12:50pm, an infant staff placed an infant on the floor immediately after the diaper change without washing his hands.
Plan of Correction: The program director provided the following plan of correction:
This child's hands were washed after she was reminded to do so. Handwashing was discussed with infant staff during this inspection.
All infant and toddler staff will be reminded of required hand washing procedures after diapering and toileting.
All staff will be re-training on all appropriated hand washing procedures at an upcoming staff training/meeting.
