Inspector Notes

An unannounced monitoring inspection was initiated at 10:20 am and completed at 2:20 pm on 01/18/2024. There were 135 children in care, ages infant - 5 years, with 22 staff supervising.

Children were observed during late morning program time which included large group and outdoor play. Lunch and nap was additionally reviewed. The inspector reviewed a sample size of 7 staff and 6 children's records.

The information gathered found one violation in the area of out-of-state background check requirements. This violation is listed on the violation notice issued to the facility.

Based on record review and interview, the center failed to obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
1. Staff 1 lacked the results of an out-of-state central registry check from another state she has resided in within the past five years.
2. Administrative staff confirmed that an out-of-state central registry check had not been obtained for staff 1.
Plan of Correction: Administrative staff stated that a sex offender check had been obtained for staff 1 as it was thought this satisfied the out-of-state background check requirement.
An out-of-state central registry check was requested, during the inspection, for staff 1. The results will be forwarded to the inspector when obtained.
