Inspector Notes
This monitoring inspection was initiated by licensing staff and concluded on 01/09/2024, between the hours of 10 am to 12:15 pm. There were 4 children present (13 points), ranging from infant to preschool-age, with 2 caregivers supervising. The children were engaged in the following: freeplay, music, song, learning, snack, video time, toy play, diapering, hand washing, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication certification; special care and emergencies; and nutrition. A total of 13 children records, and 2 staff records were reviewed.
Information gathered during the inspection determined that there were (4) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

1. On the date of inspection (01/09/2024), the following children did not have the documentation of a physical examination available for review: Child #6 (start date - 11/15/2023), Child #11 (start date - 04/17/2023), Child 12 (start date - 04/17/2023), and Child #13 (start date - 11/13/2023).
2. The Provider confirmed that all listed children were still currently enrolled.

1. On the date of inspection (01/09/2024), only 1 of 2 emergency contacts was listed for Child #5.
2. The child's record was not available for review for Child #13. Provider stated that she is still awaiting its return from the child's parent.

1. On the date of inspection (01/09/2024), documentation of the child's immunizations was not available for review for Child #5.
2. The start date for Child #5 was listed as 12/11/2023. Child #5 was in attendance on the date of inspection.
Evidence -
1. On the date of inspection (01/09/2024), the current immunization documentation on file for Child #10 was dated - 03/01/2023.
2. On the date of inspection, based on documentation on file, Child #10 was under the age of two years.
