Inspector Notes

This Subsidy Health and Safety Inspection (SHSI) was initiated by licensing staff and concluded on 07/10/2024, between the hours of 9 am to 12 pm. There were 8 children present (20 points), with 4 caregiver supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined (3) non-compliances with the applicable standards. Violations were documented on the Violation Notice issued to the home.

Standard 8VAC20-790-250-B
Based on review, 1 of 4 staff files indicated that within the first 90 days of employment, all caregivers did not complete Virginia Preservice Training for Child Care Staff sponsored by the Department of Education.

Evidence -
1. On the date of inspection (07/10/2024) it was determined that Staff #2 (start date - 12/20/23) did not complete the 10-hour Virginia Preservice training within 90 days of their start date.
2. A certificate of completion was not available for review.
Plan of Correction: This training will be obtained.
Standard 8VAC20-790-250-D-1
Based on review, 1 of 4 staff files indicated that a current certification in cardiopulmonary resuscitation (CPR) and First Aid appropriate to the ages of children in care was not completed within 90 days of their start date.

Evidence -
1. Certificates of completion for CPR and First Aid were not available for review for Staff #2.
2. The start date for Staff #2 was listed as 12/20/23.
Plan of Correction: This training will be obtained.

Based on review, Lockdown procedures have not been practiced at least annually.

Evidence -
1. On the date of inspection, the drill log was reviewed.
2. A completed lockdown drill for 2023, and to date, for 2024 was not recorded.
Plan of Correction: This drill will be conducted.
