Providers are required to obtain a sworn statement for all adult household members disclosing whether he has ever been convicted of or is the subject of pending charges for any offense within or outside the Commonwealth and whether he has been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth.

There was no sworn statement available at the time of inspection for household member #1.

Within the first 90 days of employment or service all caregivers shall complete Virginia Preservice Training for Child Care Staff sponsored by the Department of Education.

Caregiver #2, who has been employed for seven months, has not completed Virginia Preservice Training.

All caregivers shall have within 90 days of employment or 90 days from
subsidy vendor approval current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Caregiver #2, who has been employed for seven months did not have certification in cardiopulmonary resuscitation (CPR) in their file.

For assistants who are hired after June 30, 2010, documentation that two or more references as to character and reputation as well as competency were check before employment.

Providers shall maintain current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department.

The record of caregiver #1 contained documentation of CPR certification that expired seven months ago.

Providers shall maintain current certification in first aid from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current first aid certification issued within the past three years by a community college, a hospital, a rescue squad, or a fire department.

Staff #1's certification in first aid expired seven months ago.

A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for Caregiver #1 was not completed and was overdue by 22 months.
An updated TB screening for Household member #1 was not completed and was overdue by eight months.

Poisonous materials shall be stored in an area inaccessible to children.

Poisonous cleaning materials were in an unlocked bathroom cabinet and accessible to children.

Sharp kitchen utensils and other sharp objects shall be inaccessible to children unless being used by the caregiver or with children under close supervision.

Sharp knives were found in a kitchen drawer that was accessible to children.

Before the child's first day of attendance, parents shall be provided in writing the Declaration of Liability For.

Child #2 and #5, who have been in care for over one year did not have the Declaration of Liability Form in their file.

The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.

The last practiced emergency evacuation was in July 2025.

Shelter-in-place procedures shall be practiced a minimum of twice per year.

Shelter-in-place drills were not conducted at least twice during 2025.
