Inspector Notes
An unannounced inspection was initiated on 3/9/2026 and completed on 3/9/2026 in response to a complaint received by the licensing office on 3/6/2026 relating to allegations of licensed capacity. The inspector reviewed 5 children?s record, 3 caregiver, and 1 household member records on-site. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

The maximum licensed capacity of the home is 12 and there were 13 children, plus one of the provider's own children. The 13th child left during the inspection.
The provider has not taken written attendance since February 18, 2026.

3. Children from two through four years of age count as two points each;
4. Children from five years through nine years of age count as one point each;
Caregiver #2 and Caregiver #3 were observed supervising 14 children, including one provider's child, (45 points) without a third caregiver. The caregivers were over the supervision requirements by 13 points for approximately 30 minutes during the inspection.
There was no record for Child #1, who was in care during the inspection and had been in care for one week.
