Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on 3/15/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 61 children present, ranging in ages from infants to 5 years old, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 5 staff records were reviewed.

1. Staff #3 (Date of hire 1/24/2022) lived in another state (PA) in the last 5 years and had the results of a criminal history record information check (received on 1/25/2022) and sex offender registry check (received on 1/25/2022) from PA.
? Sworn Statements must be done immediately after accepting position to ensure out of state living.
Evidence:
1. Child #2 (Start date 1/31/2022) did not have a physical on file at the center.
? Parents will be provided with digital and hard copies of School Entrance Health form to
alleviate any confusion.
? Create Written enrollment instructions a cover page for School Entrance Health Form.
Evidence:
1. Staff #1 (Date of hire 3/15/2022) had a TB test or screening dated 11/18/2021.
2. Staff #2 (Date of hire 2/08/2022) had a TB test or screening dated 8/12/2008.
