Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on 3/15/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 61 children present, ranging in ages from infants to 5 years old, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 5 staff records were reviewed.

Based on record review, the center did not obtain a copy of the results of a sex offender registry check and the criminal history record from any state in which the individual has resided in the preceding five years prior to the 1st day of employment.
Evidence:
1. Staff #3 (Date of hire 1/24/2022) lived in another state (PA) in the last 5 years and had the results of a criminal history record information check (received on 1/25/2022) and sex offender registry check (received on 1/25/2022) from PA.
Plan of Correction: ? Retrain Individuals handling hiring paperwork to ensure they understand the process.
? Sworn Statements must be done immediately after accepting position to ensure out of state living.
Standard 8VAC20-780-140-A
Based on review of four children's records, the center did not have documentation of a physical in each child's file prior to or within 30 days of first attendance.
Evidence:
1. Child #2 (Start date 1/31/2022) did not have a physical on file at the center.
Plan of Correction: ? Calendar Reminders will be sent to parents and staff for School Entrance Health forms.
? Parents will be provided with digital and hard copies of School Entrance Health form to
alleviate any confusion.

Based on review of five staff records, the center did not documentation that a tuberculosis (TB) test or screening had been completed within the last 30 calendar days of the date of employment for two staff.
Evidence:
1. Staff #1 (Date of hire 3/15/2022) had a TB test or screening dated 11/18/2021.
2. Staff #2 (Date of hire 2/08/2022) had a TB test or screening dated 8/12/2008.
