Inspector Notes

A Subsidy Health and Safety inspection was conducted on 9/28/2022 to review supplemental health and safety requirements.
The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard 8VAC20-790-600-D-1
Based on record review, two staff do not currently have certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care and did not receive within 90 days of employment.

Evidence:
1. Staff #6 (date of hire 3/17/2022) and Staff #7 (date of hire 5/26/2022) do not have current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Plan of Correction: We will make sure that within 90days or less the CPR/First Aid will be
completed for new employees moving forward. We now have an
Academic Advisor who will now be in charge of training for new
employees. Both employees are now CPR certified, they had a class on
5/25/2022.

Based on record review, two staff do not currently have certification in first aid appropriate to the ages of children in care and did not receive within 90 days of employment.
Evidence:
1.Staff #6 (date of hire 3/17/2022) and Staff #7 (date of hire 5/26/2022) do not have current certification in first aid training.
Plan of Correction: All of our staff members are now certified in First Aid. We will make
sure that all new staff members are up to date within their first 90 days.
We will also make a list when they expire and will be on top of it.

Based on record review, the four staff did not take the annual safety update course.
Evidence:
1. Staff A (start date 6/20/2020) took the annual safety update course on 8/20/2021.
2. Staff E(start date 4/08/2021) took the annual safety update course on 4/15/2021.
4. Staff F (start date 6/01/2020) took the annual safety update course on 3/05/2021.
5. Staff G (start date 8/17/2021) took the annual safety update course on 9/15/2021.
Plan of Correction: Moving forward, we will make sure to do all of the necessary training on
their Orientation week. We have a new plan so that new employees will
have their training finished by their first week of working with us.
