Inspector Notes

A renewal inspection was conducted on December 5, 2022. There were five children present with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-390-B
Based on observation and interview, provider did not ensure each child has an individual location in which to keep clothing, toys and belongings.

Evidence: A child's coat was observed with household linens laying on the dining room table. Provider stated the child's coat hook had been pulled off the wall.
Plan of Correction: Coat hanger will be replaced on the wall today.
Standard 8VAC20-800-60-B
Based on review, provider did not ensure 2 of 5 children's records have documentation of required information.

Evidence: The following was found during children's record review:
1. The record for child #3 (start date 9/5/2020) did not have insurance plan # and father's employment address.
2. The record for child #4 (start date 6/1/2020) did not have an address for the second emergency contact.
Plan of Correction: Information will be requested from parents and added to record.
